MLTC programmes

Patterns of illness and demand for healthcare have changed greatly in the UK over recent years. More than 17 million adults are now living with long-term conditions, creating new challenges for healthcare services. In addition, a small proportion of these patients consume a large proportion of total healthcare resources.

The UK Department of Health has therefore made the management of long-term conditions (MLTC) a priority by declaring a public service agreement to reduce the number of emergency admissions to hospital. Apart from its intuitive logic, there is mounting evidence that MLTC can significantly decrease the number and length of hospital admissions, whilst improving quality of life for patients.

Outcome Technologies has considerable experience in the technology necessary for MLTC programmes, such as data management, event scheduling, web portal construction, and outcomes measurement and analysis tools. Our products are built within a BS7799: Part 2: 2002 environment, the NHS information authority code of practice.
